I never thought I'd have to talk about this, but... my very first semester of college I got busted for underage drinking, and had to meet with the Dean of the school and take alcohol classes and pay a fine.... But I mean, I'm 31 now, and that was 13 years ago! In fact I almost forgot about it completely... Do I really have to discuss this on my app? I can't imagine that would look good

I can't imagine that any of these minor incidents would matter to them and make them rescind your admissions, so if you don't mind the hassle of faxing in the form, it's probably fine to answer yes to the question.
